Contact our centralized scheduling line at 888-658-8483 or call the closest clinic and ask to speak to the clinic director. We will work with you to get your patient taken care of as quickly as possible.
To refer your patients, follow these three easy steps:
1. Download the Referral Pad, or use your own.
2. Fill it out with your patient details.
3. Email it to us at [email protected] or fax it to us at 833-294-8725.
We are committed to contacting patients immediately and offering patients appointments within 24-48 hours of referral.
